Notary forms and applications may be found on the Notary Forms and Fees​ page. You may also contact the Notary Division to request forms, publications and additional notary information.

Questions About Your Pending Commission?

Please note it takes about two (2) to three (3) weeks for a notary commission to be issued after the completed application and payment are received by this office. If you have not received your commission letter within three weeks after mailing your application, please contact the Notary Division at the contact information above.

Unfortunately, some letters become lost in the mail, so let us know if you are concerned that our office has not received your application.
