Complete the online Application for Appointment as a Notary Public via the Enterprise Registration & Licensing System to apply for your initial appointment or to apply for recommission. Complete the application being sure to follow the instructions and submit it to the Secretary of State with the filing fee for the initial registration or to apply for recommission.

* The $52.00 fee is waived for state and local government notaries. To have the fee waived, a letter from the head of your state or local government office certifying that the application is made for the purpose of the government office must be included with your application. Federal employees are not eligible.

Filing a Complaint

Use the General Complaint Form to file a complaint against a notary public suspected of fraudulent practices or to make a general complaint against a notary public commissioned in West Virginia.
